REQUEST FOR FEEDBACK – Understanding Transfers from Long-Term Care to Acute Care

The Bruyère CLRI has partnered with a team of researchers who are interested in learning about transfers from long-term care (LTC) homes to acute care hospitals in Ontario. The goal of this project is to identify modifiable system and LTC home factors that influence transfers to acute care hospitals.   Ultimately this research will enhance resident-centered care by informing interventions that prevent unnecessary transfers.

 

The researchers are inviting Directors of Care to complete a 30-minute ONLINE SURVEY.  Please share the link with your DOC.  Participation is voluntary.  All LTC homes that participate in the study will be entered to win one of two $300 gifts that will be given to their resident council.
