LTC?

Target audience for day two: We encourage all homes that have identified palliative or end-of-life care in their
Quality Improvement Plans or are members of the Transformational Models in LTC Working Group to send a
management /leadership representative together with a direct-teammember who is a champion of culture change
or a palliative approach to care. For other homes interested in attending, please email us.
