Long-term Care Community of Practice for Physicians and Nurse Practitioners

The Long-Term Care Community of Practice (LTC CoP) connects physicians and nurse practitioners working in Ontario LTC homes to discuss real-world clinical challenges, share practical solutions, and learn from peers and researchers. The program offers 4 accredited virtual sessions each year and this activity has been certified by the College of Family Physicians of Canada and the Ontario Chapter for up to 2 Mainpro+ Certified Activity credits (Program ID: 302726).
Our upcoming session, Quality Improvement in LTC, will take place on April 29, 2026 (7 pm – 9 pm). The session will explore practical approaches to quality improvement in LTC, including evidence-based frameworks, clinical and research perspectives, and an introduction to the ONSPARK project.
