Presenters:

Kittie Pang, BSc, MHA (CC)
Kittie Pang is the Administrative Lead for Sunnybrook’s SCOPE program and has supported its growth since 2018. With over 10 years of experience in quality improvement, implementation science, and change management, she advances primary care integration, Ontario Health Team priorities, and the North Toronto Primary Care Network. Her work strengthens connections between primary care, hospital, and community services to improve access and coordination.

Dr. Karen Fleming, MD, CCFP
Dr. Karen Fleming is an Assistant Professor in the Department of Family and Community Medicine at the University of Toronto and Chief of Family and Community Medicine at Sunnybrook Health Sciences Centre. She is also the Physician Lead for Sunnybrook’s SCOPE program. Dr. Fleming founded the Women and Babies 4P Maternal Health Clinic in 2014 and leads initiatives advancing integrated care and women’s health.

Kaiti Pucknell, RN – Nurse Navigator
Kaiti Pucknell is Sunnybrook’s SCOPE Nurse Navigator, supporting primary care providers in navigating the healthcare system. She connects primary care with specialists and community services, strengthening care coordination, continuity, and patient-centered outcomes.

  • Apply SCOPE pathways to improve timely access to specialist expertise in primary care.
  • Integrate strategies for collaborative care between primary care providers and specialists to enhance patient outcomes.
