Presenters:
Dasha Choitova, RSW – Dasha is a Transitional Services Facilitator with the Plus 45 Clinic at Surrey Place and has over 20 years of experience in Developmental Services. A York University BSW (Honours) graduate, she has worked in direct support and coordination for individuals with complex needs and dual diagnoses. At the Plus 45 Clinic, she focuses on supporting aging adults in the sector, addressing service gaps, and promoting cross-sector collaboration. She also co-facilitates Effective Specialized Response training in the Toronto Region. Her practice is grounded in a person-centred, strength-based, bio-psycho-social approach that integrates community and natural supports.
Angie Gonzales, RN, MN – Angie is a Nurse Professional Practice Lead and Clinical Nurse Specialist at Surrey Place, and an Adjunct Faculty member at the University of Toronto’s Lawrence Bloomberg Faculty of Nursing. She holds degrees in Life Sciences, Nursing, and Community Health from the University of Toronto. Her previous clinical experience includes work at Holland Bloorview Kids Rehabilitation Hospital in acquired brain injury and neuromuscular care. At Surrey Place’s Plus 45 Clinic, she provides nursing consultation for adults with intellectual and developmental disabilities transitioning into later life. She also contributes to the ECHO Ontario AIDD Brain Health program and the IDD Palliative Care Network.
Learning Objectives:
By the end of the presentation, participants will be able to:
- Explore the importance of how unique the experience of menopause is for every individual with Intellectual and Developmental Disabilities with the goal of creating a personalized care plan
- Be able to advocate for strategies and identify resources in creating a care plan for aging with grace
- Engage in a discussion examining some of the gaps and resources
