Strengthen your LTC home’s palliative approach to care

The Collaborative Project brings together regional and provincial palliative care experts to support Ontario LTC homes in strengthening their palliative approach to care and improving the quality of life of residents, families, and healthcare providers.

Through a free-to-join, flexible, 9-month collaborative experience, participating LTC homes receive:

  • Flexible involvement that supports practical application
  • Tailored education and coaching based on your homes’ needs
  • Implementation expertise to help translate knowledge into practice
  • Opportunities to engage team members and family representatives
  • Established sector partnerships bringing together regional and provincial palliative care expertise
