Join us at the ‘This is Long Term Care 2025’ Conference from October 26-28, 2025 in Toronto. Get to know our project teams and leaders from all three Ontario CLRI host centers. We are excited to share our knowledge through our engaging sessions, provide you with new resources, and hear from you on LTC sector improvements. Engaging sessions:

We have three workshops and presentations planned that tackle important issues in long-term care. Our sessions will take place all day on Monday, October 27, 2025in room 200A. Don’t miss them!

10:45 – 11:30 a.m.: Living Classrooms: A Model for Transformative Personal Support Worker Education in Long Term Care Homes

This session will showcase the successes of current Living Classroom partnerships, drawing on formal evaluation data, early research findings, and a range of resources developed to support each phase of the partnership journey. Participants will gain valuable insights and practical strategies to help integrate a Living Classroom model into their Long-Term Care home—covering the full spectrum from initial concept and planning, through implementation, to ongoing evaluation and long-term sustainability.

2:15– 3:00 p.m.: Shaping the Future of Dementia Care: Insights from Ontario’s Long-Term Care Homes

How are long-term care homes in Ontario delivering—and envisioning—dementia care? Join the Ontario CLRI as we share insights from the Dementia Care Preparedness Initiative, with input from 301 homes across the province.

Explore current practices, emerging innovations, and shared aspirations for the future. From key trends to common challenges, this session offers a snapshot of Ontario’s evolving dementia care landscape—grounded in the collective voice of those on the front lines.

3:15 – 4:00 p.m.: Creating a Grief-Friendly Workplace in Long-Term Care

This session explores why cultivating a grief-friendly workplace is not only essential for staff wellness and retention, but also foundational to delivering meaningful, person-centered care.

Discover how acknowledging and supporting grief builds a more resilient LTC workforce. Hear firsthand insights from All-In Palliative Care – The Team Approach to LTC workshops, where frontline staff shared what they truly need to feel supported in a grief-informed environment.

Leave with practical, proven strategies already in place in Ontario homes—strategies that help teams, residents, and care partners navigate the ongoing realities of loss with compassion and strength.
