Creating Quality Interprofessional Teamwork for Serious Illness Conversation and Complex Chronic Progressive Disease
Date & Time: Wednesday, October 28, 2026 at 11:00 a.m. to 11:45 a.m.
Location: Room #206 C
This hands-on workshop brings real long-term care scenarios to life, giving you a chance to experience interprofessional care in action — especially when it matters most. You’ll explore how PSWs catch what others might miss, how nurses translate clinical complexity into clear next steps, how social workers support families through difficult moments, and how physicians tie it all together. Through scenario-based discussion and guided practice, you’ll build confidence in recognizing early signs of decline, communicating consistently across your team, and approaching serious illness conversations with clarity and compassion. Leave with a renewed sense of what your team is capable of — and practical tools to help you get there together.
Award moments:
We are excited to recognize the extraordinary efforts of a person-centred language (PCL) champion during the conference. Could you be our next winner of the PCL award?
