Join Pallium’s new Community of Practice (COP) designed for interdisciplinary healthcare professionals working in Ontario’s long-term care sector. If you are actively engaged in quality improvement (QI) initiatives related to palliative care within your long-term care (LTC) home, this COP is for you!

About this Community of Practice

This COP aims to support healthcare professionals in successfully planning and implementing palliative care-related projects under your LTC home’s provincially mandated Quality Improvement Plans (QIPs). This COP is ideal for individuals who are action-oriented, have a foundational knowledge of QI methodologies, and have a basic understanding of the principles of a palliative care approach.

Through this Community of Practice, you will have the opportunity to:

  • Share and Learn: Exchange lessons learned from previous QI projects and receive valuable feedback from your peers.
  • Gain Expert Insight: Benefit from advice and guidance provided by both content and context experts in palliative care and quality improvement.
  • Build Shared Resources: Collaboratively develop tools and resources to support and enhance palliative care QI initiatives.

With a focus on equity and practice-level case-based discussions, COP members will work together to strengthen the foundational practices needed to support high-quality, inclusive palliative care for all individuals.

November 26: Integrating Equity, Diversity, and Inclusion in Palliative Care Quality Improvement in Long-Term Care

Description: In this session, presenters will highlight how discrimination, bias and systemic inequities impact access and provision of palliative care in long-term care homes. We will explore through interactive exercises with case examples how equity, diversity, inclusion and belonging principles can be incorporated into palliative care quality improvement plans to help support inclusive, culturally responsive palliative care delivery. Presenters will also share a suite of resources and learning opportunities that have been co-designed with and for LTC homes to support teams to provide respectful, dignified and inclusive care of the diverse populations they serve.

Learning Objectives:

  • Recognize how discrimination, bias and systemic inequities can affect access and provision of palliative care in long-term care.
  • Describe practical strategies to integrate principles of equity, diversity, inclusion and belonging into ongoing or planned palliative care quality improvement initiatives in LTC.
  • Learn about resources and learning opportunities available from the Ontario CLRI to support capacity-building in equity, diversity and inclusion in LTC.
