Welcome to the Aim, Align, Achieve Quality Palliative Care registration form!

What is this class?

This 4-hour, virtual, interactive program that we are offering to Ontario LTC homes for education that supports quality improvement and change management in the context of improving palliative care.
This class is for quality improvement committees; palliative care committees; physicians; quality improvement coordinators; best practice advocates; and everyone who wants to work on a person-centered approach to care that honors the humanity of residents, team members and families.
Each session is limited to 30 learners – please register soon!
Wage support is offered to participating learners – $30/hr for LTC team members and limited scholarships available for physicians at $100/hr

Before completing this form, please ensure you meet the following criteria:

You are your LTC home’s Administrator or you are filling out this form on behalf of your Administrator.
We will ask you for names and emails of your learners to register, however before the day of the training, you can swap out your learners as needed.

By September 2022, we will confirm your training date(s) and follow up with next steps on how your learners can enroll in the training. There will also be a follow-up discussion session on Thursday, June 11, 2026, from 12:00 – 1:00 PM EST. All participants are invited to attend, and a link to join will be shared at a later date.

Each learner needs to connect on their own screen. A tablet, laptop or computer connected to Wifi, audio and webcam is required for each team member to attend the training. Team members cannot attend together using one screen.
