Person-Centred Care in LTC Community of Practice

The Champlain Region Person-Centred Care in LTC Community of Practice is expanding to a provincial audience! Join us for the Person-Centred Care in LTC Community of Practice (PCC in LTC CoP), hosted by the Ontario CLRI at Bruyère Health. This Community of Practice (CoP) is open to all LTC team members and leaders in Ontario. The PCC in LTC CoP promotes person-centred care through shared learning, peer support and the spread of innovative, evidence-informed practices. The goal of this CoP is to support personhood and advance culture change, shifting away from an institutional model of care.
Team members and leaders will connect with peers across the province to support each other on their person-centred care or emotion-based care journeys. This group is open to those who are at any point of their culture change journey, including those looking to start their journey to those who are looking to strengthen their current PCC initiatives.
Benefits of joining this CoP include:
- Connecting with others across LTC homes in Ontario, and sharing activities, learnings, and identifying ways to support each other
- Hearing from expert speakers on specific topics of interest
- Bringing forward specific issues or questions relating to their home’s person-centred care journey, and having support and feedback from peers
- Having opportunities for members to present on creative initiatives or programs they have developed or implemented
- Having access to a library/repository of CoP meeting summaries and related person-centred care resources
- Providing opportunities to facilitate new connections between people working in LTC homes, sector partners, and other subject-matter experts
The PCC in LTC CoP is guided by an Advisory Committee of LTC leaders, Champlain Region Family Council Network and our provincial partners Ontario Association of Residents’ Councils, Family Councils Ontario, and Ontario Caregiver Organization.
